Abstract
The failure mode, effect and consequence analysis (FMECA) has been widely used to perform the risk magnitude assessment. In a conventional approach, three parameters (severity, occurrence, and detection) are utilized to describe each failure mode by rating on a 1-10 scale, whose product results in the risk priority number. This paper presents a method for assessing the risk priority of a dam failure based on fuzzy theory approach. There are several difficulties during conducting conventional FMECA, such as the subjective and qualitative scale in linguistic description, important among the risk ratings, and the difference of risk representation among the same ratings. Hence, a fuzzy assessment system is proposed in this paper to deal with these difficulties. This study is applied for the Simplicio hydroelectric power plant, located in the Southern region of Brazil. The results indicate that by using the proposed methodology the risks associated with dam failure can be assessed with more in-depth technical input
